What Type of Report Are You Submitting?

VA Accountability Tracker (VAT) Report:
Submit this if you are a veteran, patient, or caregiver reporting issues such as delays, privacy violations, staff misconduct, denial of care, retaliation, or unethical treatment. These reports help identify patterns of harm within the VA system. You should file this after you have attempted to get help from the VA and they have failed to provide it.

Whistleblower Report:
Choose this if you are an employee, contractor, or insider reporting fraud, abuse, retaliation, or serious policy violations within the VA or related systems. This option supports anonymous submissions and protects your identity.
